Request prescriber portal access
The portal has our order forms, prescriber guides, compounding base studies, fax cover sheet and EMR instructions. Tell us who you are and our pharmacist will review your request.
How it works
- Fill out the form. Prescribers enter their NPI. Office staff enter the NPI of the prescriber they work for. Veterinarians enter their license number instead.
- We check the NPI. Your NPI is checked right away against the national NPI Registry.
- Our pharmacist reviews it. You'll get an email when your access is approved.
- Sign in with your email. At the portal, enter your email address and we'll send you a one-time code. There's no password to remember.
Already approved?
Need access sooner, or have a question? Call the pharmacy at 626.631.6150. You can always send prescriptions by e-prescribing, fax or phone; portal access isn't required to order.
