Ask your provider about compounding

Compounded medications need a prescription from a licensed provider. If your provider hasn't prescribed a compound before, this page and the printable sheet below explain how it works.

Before your appointment

  • Write down what you've tried and why it didn't work for you, such as a strength you need that isn't made, an allergy to a dye or filler, or a dosage form you can't use.
  • Bring the printable sheet below to your provider.

What your provider decides

Whether a compounded medication is right for you, and the ingredients, strength, dosage form and directions. We prepare exactly what your provider prescribes.

How your provider sends it

E-prescribe to EpitomeRx, Inc. (NCPDP 5689584), fax 626.631.6151, or call 626.631.6150. Prescribers can find order forms and EMR instructions on our For Prescribers page.

After we receive it

We call you to confirm details, your price (shipping included) and timing before we prepare anything.
